Established Patient Portal Info

If you are a solo adult patient you will want to use your regular email address to register for the portal.


If you are the parent of a minor child, you must have a DIFFERENT/SPECIAL or ALIAS email for your child or each child.
You, as the adult, may then manage one or ALL of your children under one account with your email address.
For example, smithfamily@gmail.com is the parent's email address.
You can use jonny.smithfamily@gmail.com to register your child.


Log into the New Patient Portal one last time below and give us the parent's name and email and the child's name and email to gain access to your child's information.
